Admissions Checklist

Little Madison Academy Primary School will serve PK4 through first-grade students; Madison Academy Elementary School will serve second through sixth-grade students. Madison Academy Upper School will serve seventh through twelfth-grade students. 

New families applying to Madison Academy must create an account. Once the account is created you will receive login information to access the online application. 

Returning or current families may navigate directly to your parent portal to add a prospective student and access the online application. 

Admission Application Checklist

  • Upload Student Headshot Photo and Unofficial Records Request form
  • Fill Out Parent Questionnaire, grades PK4 - 12 
  • Fill Out Student Questionnaire, grades 7 - 12 
  • Gather and Input Reference Emails
  • Schedule a Student Shadow Day 
  • Schedule an Entrance Exam and Principal Interview (Observation for grades PK4 through 1)

Students in grades 1 through 12 are invited to spend a day shadowing a student on campus if applying during the school year.  After you have submitted an application, we will match the student with a Madison Academy student who will be their tour guide for the day. Your student will also take a placement test and interview with their school principal on their shadow day. 
 

 

Once you have completed the admissions requirements, the Admissions Team will schedule a school-readiness evaluation for students applying for grades PK4 and Kindergarten. Students applying for grades 1 through 12 will complete an entrance exam. This is normally completed on their Shadow Day. 

 

Once you have completed the admissions requirements, the Admissions Team will schedule a time for us to get to know you and your child. Students applying to PK4 through first grade will go through an informal family interview with the Little Madison Academy Principal. The Elementary Principal will conduct a family interview with students in grades two through six, while students in grades seven through twelve will have an interview with an Upper School Administrator. 

 

Once our Admissions Team receives all the required documentation, ​we will create a profile for our Admissions Committee to review. Students are assessed on the following:  

  • Student Interview 
  • Referral Forms - two academic recommendations and one personal/character reference
  • Transcript or Report Card and Standardized Test Results (if applicable)
  • School Readiness Assessment (grades PK and KG)
  • Entrance Exam (grades 1 through 12)
  • Shadow Day Report
